Planning to Travel Abroad?  Plan to Get Your Passport now!

 Prepare for your upcoming travel needs and apply for your U.S. Passport at the Madison County Probate Court, located at 100 North Side Square, Huntsville, AL 35801. 

All U.S. citizens planning international travel may apply for their passports at our offices, starting on Tuesday, September 3, 2019 from 8:30 am through 4:30 pm, Monday through Friday.

To apply for an initial Passport, you must apply in person using Form DS-11.  A fillable form is available for your use on the US Department of State’s Website at or by using this link:  Form DS-11

The fillable Renewal Passport Form, DS-82, and many other fillable, Passport related forms can also be obtained directly from US Department of State’s website by using their Forms link.  The DS-11 Passport Renewal Form cannot be executed by our Passport Acceptance Agents and must be completed by and mailed directly to the US Department of State by the applicant.  All mailing addresses are listed on the last page of the application. For your convenience, our office carries all Passport related forms and they are available upon request.
