All U.S. citizens planning international travel may apply for their passports at our offices during the following times: 8:30 am through 4:00 pm.

Monday, Wednesday, and Friday: By appointment only. 8:30 am through 4:00 pm.

Tuesday and Thursday: First come, first serve from 8:30 am through 4:00 pm.

Free fillable passport forms, passport fees, and information on how to apply can be found on the Department of State’s official website for passport and travel information, provided with this link: US Department of State - Passports. Please note that any other website offering passport forms or help filling the forms out for a fee, does not mean you are exempt from paying the application fees when submitting your passport.

Fees cannot be paid with debit or credit cards. The Department of State’s passport fee must be paid by check or money order.

 The $35.00 Execution/Acceptance fee must be paid by check, money order, or cash.  

To apply for an initial Passport, you must apply in person using Form DS-11.  A fillable form is available for your use on the US Department of State’s Website at travel.state.gov or by using this link:  Form DS-11. 

The fillable Renewal Passport Form, DS-82, and many other fillable, Passport related forms can also be obtained directly from US Department of State’s website by using their Forms link.  The DS-82 Passport Renewal Form cannot be executed by our Passport Acceptance Agents and must be completed by and mailed directly to the US Department of State by the applicant.  All mailing addresses are listed on the last page of the application. For your convenience, our office carries all Passport related forms and they are available upon request.
