Since 1918 the Health Department has been working in both the city and county to protect and improve the health of area residents and to ensure a healthy environment in which to live. The department employs public health nurses, environmentalists, health educators and nutritionists to provide health care to individuals, children, families, the school and the community.

Birth Certificates - If born anywhere in Alabama, you can fill out a request form at the Madison County Health Department and obtain a duplicate birth certificate within about 30 minutes. $15 for first copy, $6 for each additional.

Death Certificates - If death anywhere in Alabama, a request form can be filled out at the Madison County Health Department and obtain a duplicate death certificate within about 30 minutes. $15 for first copy, $6 for each additional.

or contact Vital Statistics, Montgomery, AL at 334-206-5418. You can request by phone and use payment by credit card. Overnight mailing is available.

WIC Nutrition Program

WIC is a federally funded program that provides free food and nutritional education to pregnant and breast feeding women, children up to age five, and free baby formula to infants. Nutrionists and nurses assess each applicant and tailor a food and counseling package to meet that person’s needs. Information about the program and what it takes to qualify can be obtained at this office.
